Ottawa Carelton Futsal League Callup Form


Please copy this form and email your coordinator(email found 
on Contacts page, futsalreg and webmaster
(include @futsalottawa.com to ends of each id above).  
We must receive this form via email or fax(596-0211) 
24 hours prior to your game.  The league coordinator
will find a player and approve the call-up via email with the name
of the person to be called up.

Approval of the callup will be made by email.  Place the persons
name on the gamesheet at game time and have the person initial it.    
Bob Rathwell will then place the call-up into our system.


By sending the above form you acknowledge that you agree with the
following terms:


    1)      Your team will have 7 or fewer players from your original
    roster.  The only exception is if you do not have your goalie a
    goalie may be called up. (Youth U14 and below and Women’s Div 3 and 4 may not use this exception)

    2)      A player can only be called up 3 times per season(goalies will be exempt from this rule)

    3)  A maximum of 2 call-ups per game.
    4) The player called up will be from a lower division than
    yourself

    Men Div 3
    Women Premier
    Coed Div 1
    Coed Div 2
    Women Div 1
    U18 Divisions(some players may play in Women div 2 and 3 with
    approval from Coordinator)
    Women Div 2
    Women Div 3
    U16 Divisions
    ....
    U8
